We would like to hear from you! Share your voice in the 2021 Granville Vance Public Health Community Health Survey. This is your chance to let us know what is most important for the health of your community. If you received a postcard in the mail, enter your postcard number. If you didn't receive a postcard, don't worry you can still participate!
After completing the survey, you’ll have the chance to enter a raffle for $150.
