TOWN OF STEM: September Is Pace (Programs For All-Inclusive Care For The Elderly) Month

Adultcare

Town of Stem issued the following announcement on Sep 25.

Did you know that some Granville County residents are eligible to receive PACE services to address health, respite, therapy, specialty care, meals, home health, vision, dental, mental health, transportation and end of life care? These services, which can be provided in the home or at a PACE Center, have several payment source options. Residents may also be able to receive these services through their insurance.

Granville County's Department of Social Services urges residents age 55 or older who

reside in their PACE organization's service area (some sections of Granville County apply) to check eligibility requirements. The closest PACE center to our area is Senior Community Care of NC, located in Durham.

Click here for more details from the NC Department of Health and Human Services: https://medicaid.ncdhhs.gov/.../program-all-inclusive... or contact the Senior Community Care of NC at 919-425-3050 to speak with an enrollment specialist.
